In conjunction with disease management initiatives, employers and health plans can now utilize our Diabetes Self-Care Reinforcement™ program to add to their efforts to keep employees/members healthier, reduce sick days and reduce overall health care expenses. Unlike most diabetes disease management programs which focus the majority of their resources on the patients who have become the most sick, Neighborhood Diabetes focuses on teaching preventive self-care and monitoring adherence early on after diagnosis. Our goal is to stem the rising tide of patients who are becoming hospitalized with the traditional heart, circulatory, kidney, vision or other common problems resulting from diabetes and keep them functioning at healthy levels for longer periods of time.
Our program is once again built around our "high touch" approach to diabetes preventive care. Or as we like to say, diabetes self-care is a contact sport.
The Home Visit
We begin our interaction with most new patients or employees with diabetes via a conveniently scheduled home visit where we begin the early reinforcement of proper preventive care. The visit typically lasts about an hour and focuses on the following:
- We review the recommended diabetes preventive care protocol both verbally and in writing (blood glucose self-testing, HBA1c testing, foot exams, eye exams, lipid and microalbumin tests, flu and pneumonia shots)
- As we do with all of our new customers, we provide hands-on training on effective blood glucose testing (selecting an appropriate glucose meter, drawing blood with minimum discomfort, ongoing meter calibration and maintenance, properly recording blood glucose readings).
- We review the services and reimbursement coverage of the relevant health plan (commercial or Medicare for retirees) so that full advantage is taken of the resources offered to help stay healthy.
Follow Up Phone Contact
We match your employee or member with a Neighborhood Diabetes customer care representative who remains his or her contact person throughout and, hopefully, becomes a trusted resource. Matches can be made according to language where necessary. We currently offer English, Spanish, Portuguese, Russian, Cambodian (Khmer) and Haitian Creole with more languages in the works. The follow up phone calls are made, at a minimum, once a quarter to handle the following:
- We check to see if your patients need to replenish their supplies (testing strips, lancets, control solutions, meter batteries), including medications if we are serving the patient through our Neighborhood Pharmacy.
- We check to see if they are having any issues which prevent them from testing at the frequencies prescribed by their physician. If your employee/member is struggling, we will again visit his/her home to re-train him/her on the mechanics of testing.
- We follow up with their progress in completing their office visits for HbA1c’s, foot exams, eye exams, etc. as outlined during the initial training, encouraging them to take action where they have not yet done so.
- We try to establish a personal rapport so that they feel comfortable calling us if they need further support.
Information Exchange
We are able to provide customized information to any employer or health plan desiring to track either an individual’s, population’s or provider’s ability to meet the objectives of the preventive care program.
